What do you need to bring to your appointment?
- Preferably a filled-out Medical History, which you can find here. If you are not able to fill this out beforehand, you may want to come in 5 minutes early to your appointment to do so.
- Your health fund card, if you have one. We can only claim by swiping your card.
- Any relevant dental documents if you have come from another dentist.
Parking is located onsite or across the road.
Public transport via Busses is also available on Maluka Drive for the following bus services: 70 71 72 73 77.
We have HICAPS available in the practice. We do need your physical health fund card to be able to claim for you on the day of your appointment. Alternatively, we can issue you a receipt with the relevant item number, which you can take to your health fund.
